Interim MDS Coordinator (Remote and/or Onsite)

Polaris Group, an Assembly Health company, is looking for interim MDS Coordinators who will partner with our organization to meet our objectives and help manage our client partnerships.

We are looking for MDS Coordinators who can work remotely and/or onsite.

What our MDS Coordinators do:

  • Participate in patient assessments by overseeing the assessment process, setting schedules, and assuring accuracy.
  • Determine accurate expenses associated with a potential admission.
  • Review all applications for admission and communicate results of the assessment.
  • Complete and assure the accuracy of the MDS process for all residents.

 

  • Maintain current working knowledge of criteria, serving as a resource for staff and communicating changes in regulations.
  • Look for potential risks and/or changes that may affect reimbursement.
  • Facilitate problem-solving for complicated admissions.
  • Start Medicare process and monitor assessment schedules and nursing documentation to ensure accuracy and timely submission.
  • Maintain client confidentiality and function within HIPAA regulations.
  • All other assignments and projects as directed by leadership.

 

What it takes to join the family:

  • Experience in MDS coordinator role or related experience.
  • Current and valid RN license in compact and/or non compact states
  • Experience in a clinical and healthcare setting. Deep medical terminology knowledge.
  • Superb decision-making and problem-solving skills. Superior attention to detail.
